| Notes on evidence: | Lambeth Palace 192 fols. 1–44 and 188 fols. 175–179 were formerly bound together (by Bale?). Sancroft removed five leaves to another volume, but failed to remove with them the leaf still remaining in MS. 192 bearing the scribble "quod R [...]", which shows that it is from the same MS. as MS. 188 fol. 175r on which is written "quod R Gorst" in the same hand.
